HOW TO ORDER

For your convenience, there are now 3 ways to order your prescriptions...

1. For new prescriptions, have your physician call in or fax your prescription directly to our pharmacist. Your physician can obtain a prescription order form by clicking here.
2. To refill orders by phone, call one of our patient service representatives who will take your order in just a few short minutes.
3. To refill orders online, simply click here to register or login to your account. Once you have completed the registration process you may then refill your prescription. You will need your Foundation Care prescription number, product number or product name which can be found on your prescription label or packing slip. The online refill service is available only for prescriptions that have been previously filed by Foundation Care.
