VIVITROL®
VIVITROL® is indicated for the treatment of
alcohol dependence in patients who are able to abstain from alcohol in an outpatient
setting prior to initiation of treatment with VIVITROL®. Patients should not be actively
drinking at the time of initial VIVITROL® administration. Treatment with VIVITROL®
should be part of a comprehensive management program that includes psychosocial

For your patients that have been prescribed Vivitrol®, Foundation Care can assist you and
your patients by providing the product and reimbursement assistance. Once an order is received our reimbursement representatives will perform a claims analysis and notify you
and your staff. Upon approval of coverage, Foundation Care will deliver the Vivitrol® to your
office or injection site.
